NY HEALTH INSURANCE AND THE AFFORDABLE CARE ACT
Information you will need to complete an application:
- Date of Birth
- Social Security Number
- Tax Filing Status (ex. single, married filing jointly or separately, dependents, etc).
*Note, individuals do not have to have filed taxes before to complete an application on the Marketplace.
- Income information
- Name/address of current employer
- If applicable, immigration number (ex. Green card number, employee authorization number)
- Information about any other health insurance you or your family member may have
If you are meeting with a Health Navigator, you are not required bring in physical copies of any of the above documents, it's just helpful for them to know this information. Of course, people are free to bring any paperwork they feel will help them complete the application.
Other information to keep in mind when choosing a health insurance plan: Any preferred medical providers, specialists, clinics or hospitals; a list of medications and/or any other medical needs.

Note: You must have an email account to enroll online because it is used to create a NY.gov ID. Visit http://tcpl.libguides.com/email to learn how to create an email account. If you are having difficulty registering for an email account, a Health Access Navigator with the Human Services Coalition of Tompkins County will assist you in setting up an account during a scheduled appointment (see "In Person" below). Also, you can still set your preferred method of contact (for future communications from NYSOH) as physical mail if you do not regularly use email.

New York State of Health Tax Credit and Premium Estimator
The Kaiser Family Foundation also created a national calculator that determines premiums and subsidies for people purchasing insurance on their own in new health insurance exchanges.
